Horizon Center
The Horizon Center provides diagnostic, habilitative, medical and counseling/educational services for children who need specialized evaluations and a multidisciplinary approach to their care.
The Horizon Center's multidisciplinary "clinical team management" is designed to maximize children's ability to function and their quality of life. The Center houses a number of distinct clinical activities:
- Behavior Clinic
- Childrens Evaluation and Rehabilitation Center (CERC) at CHAM
- Congenital Disorders Center
- Craniofacial Center
- LINCS: primary care for children with special health needs
- Otolaryngology (ENT) Services
Each of these independent programs has access to a core group of professional staff who are key members of the treatment teams. These include Social Workers and Case Managers, a Clinical Psychologist, Nursing support, and an Administrator who organizes these services across the different clinical activities. There is also dedicated administrative support for reception, registration, scheduling and billing.
Case Managers work closely with the clinical staff to ensure that families have access to the services they need, and receive appropriate follow-up. Case Managers:
- Work with families to schedule patient appointments.
- Meet with families to review treatment status, outstanding issues, and to identify areas requiring liaison with clinical, administrative or external staff.
- Meet with families on an as needed basis for crisis intervention and/or other immediate needs.
- Document patient/family interactions.
- Assist families with insurance and billing issues.
- Troubleshoot and advocate for families and patients in their effort to obtain needed resources and services.
